We understand that you may have observed a damaging hailstorm, or may have
seen damage caused by a hailstorm, and would be very grateful if you could provide
further information by completing and returning this form with details of the event.
This questionnaire should also be used if large hail fell but no damage occurred
because there was nothing to damage. Your cooperation is very much appreciated and
may be of considerable help to our research.
